Blue Ridge Health is currently seeking a Community Health Worker to be part of our Community Outreach Team in Haywood County, NC.

What You'll Do:

The Community Health Worker (CHW) is a front-line public health worker that works as a liaison between BRH and the communities they serve. CHW’s serve as ‘ambassadors’ for BRH helping to increase access to health care and community resources in the communities they serve so as to improve health outcomes. Responsibilities include:

• Connects individuals to health care services.
• Provides basic health education/coaching to the community in order to prevent negative health outcomes.
• Provides resource navigation assistance in order to meet individual’s needs.
• Provides Street Outreach to individuals experiencing homelessness, connecting them to health, social, and housing resources.
• Helps plan, organize and staff’s community health education and screening events. Occasional evening and weekends may be required.
• Takes basic vital signs and becomes competent in basic health screenings.

What We're Looking For:

• Must possess a strong passion for community engagement work.• Lived experience or prior work experience with homeless populations is preferred.• Ability to communicate clearly and professionally in order to facilitate collaboration and cooperation both internally and externally.• Must be reliable and flexible in order to meet both the internal and external changing needs of the work.• Strong organizational skills and the ability to work autonomously.• Effectively manage assignments and projects.• Ability to prioritize and respond to multiple requests within requested time frames.• A valid driver’s license is required.• Strong interest in working with an interdisciplinary, non-profit healthcare team as well as culturally and socioeconomically diverse community and patient population.• Proficiency with Microsoft Office programs and Electronic Medical Records.
